All new participants must complete this registration form before scheduling a CDA program. The information you provide helps our staff plan safe, supportive, and successful experiences.
A parent, guardian, or caregiver may complete the form on behalf of a participant.
Please allow enough time to complete the form in one sitting. If a question does not apply, select the closest option or enter “None” where appropriate. CDA staff are happy to help if you have questions.
What Happens After You Submit The Registration?
After submitting the registration form, please complete the required participation and media waivers. These waivers must be completed before scheduling any activities.
After completing the waivers, CDA staff will review your registration to better understand support needs, safety considerations, and program interests.
If you would like help with program fees you can complete a Financial Assistance Application.
Once your registration and waivers are complete, call the CDA office to check availability and schedule activities. This process helps ensure participants are matched with appropriate equipment, volunteers, and program opportunities.
