Review and Sign Waivers
To participate in a CDA program, all participants must review and sign the Participation Waiver. The Media Waiver is optional but strongly encouraged, as it allows CDA to share photos and videos that help support our programs.
Once your waivers are complete, please call the CDA office to check availability and schedule activities. 970-257-1222
